Team PH Unmet Needs Patient Impact Fund is OPEN

July 30,2018

We are thrilled to announce the re-opening of the Team PH Unmet Needs Patient Impact Fund. This fund is designed to address the unmet needs of people living with PH through a series of direct-to-patient micro-grants of $500 that match the immediate unmet needs in their life. These are non-medical, but critical needs of those living with PH.

In the fall of 2017, with the help of our friends at PHaware, Blue Lips Foundation, Reata, and Gilead, Team PH created and opened the Unmet Needs Patient Impact Fund to PH patients in Texas, Florida, and Puerto Rico who were severely impacted by Hurricanes Harvey, Irma, and Maria. Patients were able to put these funds toward temporary lodging, gas, repairs to their home, and other non-medication needs.

We have been without power for a whole we and counting. We lost all of our food, and I can only run a generator to power a fan for us to keep cool.

Funds will be awarded at a first come, first served basis, and will be awarded until funds run out. Those interested in applying should visit the program page at www.teamphenomenalhope.org/unmet-needs-patient-impact-fund. Applicants must meet the qualifications listed on this page in order to receive a grant. For questions, please email the program manager at katie@teamphenomenalhope.org.
