Your personal information and privacy is important to Team PHenomenal Hope (“Team PH”, “we” or “us”). This privacy policy (“Privacy Policy”) describes what information we collect about you, including when and how we collect it; how we use it; with whom we may share it; and what choices you have regarding our use of your information. We will not share, sell, or trade your personal information with third parties for our own enrichment or material gain. This Privacy Policy applies to all information received by Team PHenomenal Hope, online and offline, from any Platform (“Platform” includes the Team PHenomenal Hope website, Just Giving and Classy websites, and Google web forms), as well as any electronic, written, or oral communications.

What Information We Collect

When you interact with Team PH or visit our Platform, we may collect your Personal Information. In this Privacy Policy, Personal Information means information associated with or used to identify or contact a specific person. 

The type of information we collect will depend on how you choose to interact with us. For example, if you are a donor, we will collect different information than if you are a patient sharing their story. In general, we may collect the following Personal Information about you:

Your Name and Contact Information. We may request, or you may provide, your name, mailing address, telephone number and email address. We will use this information to contact you with information, events, donation opportunities, and other communications relevant to our mission. 

Demographic Information. We may request your demographic information, such as your age (including birthdate) and your race. We use demographic information to better understand the community we serve and to share PH stories on our Platform.

Health Information. We may request health information including your pulmonary hypertension diagnosis date, your treating physician’s information, your treatment, and other information, as needed for our programs. We will use this information to confirm your eligibility for certain programs, provide relevant communication and support, and share PH stories. 

Financial Information. If you make a donation to Team PH or apply to any of our programs offering financial assistance, we will collect the personal information required to complete the transaction or consider you for financial assistance. We use third-party vendors to process donations made on our Platform. For financial information we collect and use, we will retain your information for only as long as required to process the transaction.

Social Media Information. We may collect social media identifiers and/or interests or opinions expressed if you engage with us directly on social media platforms.

Employment Information. If you apply for employment with us, volunteer your professional services, or participate in any of our programming targeted to health providers, we may collect employment information including job title and organization with whom you work.

Internet Activity and Device Information. In some instances, we automatically collect other information about you which does not directly identify you, but which may correspond with you or a particular device. We use this information to learn more about how our Platform is used and to otherwise improve and administer our online resources. Automated technologies may include the use of web server logs to collect IP addresses, “cookies” and web beacons. How We Collect Your Information

We collect your Personal Information when you: 

  • Visit the Platform;
  • Make a donation;
  • Create a profile on the Platform;
  • Volunteer with us;
  • Register for a Team PH event or service;
  • Engage in a Team PH support group;
  • Make an inquiry;
  • Apply for employment; or
  • Contact us directly through the web or in person.

We may also collect other information about you from publicly available sources or third parties to improve our fundraising campaigns and development activities. We may combine the information we receive from you with this information.

What Information We Share

We do not sell, share, or otherwise disclose your Personal Information, except as provided in this Privacy Policy.

We and our service providers may use Personal Information for the following purposes:

Support for Our Mission and Operations. The collection, use, and disclosure of your Personal Information will always be done for the purposes of furthering our nonprofit charity mission, to improve the quality of life for those living with pulmonary hypertension. This may include sharing your Personal Information publicly on our website or social media channels, such as through our PH stories.

Data Analytics. We use data to evaluate engagement levels with our Platform and programs. This analysis can help us improve user experience, mission delivery, and support. Where possible, we perform data analytics with anonymized or aggregated information that no longer identifies any individual.

Targeted Fundraising. We may use certain information (name, address, telephone number or email, date of birth, gender, or other Personal Information) to contact you for the purpose of raising money for Team PH. Any such communication will provide you with the opportunity to opt-out of receiving similar future communications.

Other Business Purposes. We may use your Personal Information for other business purposes, such as conducting audits, fraud monitoring and prevention, recordkeeping, developing new services, improving our existing programs and services, identifying usage trends, planning for and improving the effectiveness of our fundraising campaigns and mission activities, and updating or expanding our business activities.

With your Consent. In addition to the other uses described in this section, we may also use your Personal Information as you expressly authorize us.

With Whom May We Share Your Personal Information

Team PH does not sell your Personal Information. We may share your Personal Information with our service providers, program volunteers and other third parties that assist Team PH in carrying out our mission. In furtherance of our mission, your Personal Information may be shared with the following categories of third parties: service providers, mission collaborators, volunteers, and others with your consent or as required by law.

Third-Party Service Providers. We may need to share Personal Information with third parties that provide operational services on our behalf (such as website hosting, information technology and infrastructure, data analytics, email delivery, auditing, or other services) or that assist us in providing you with support and resources.

Team PH Affiliates. We may provide your information to affiliated organizations that share a common mission with Team PH.

Mission Collaborators. We may also collaborate with organizations to provide or improve Team PH’s services and operations. 

Volunteers. We may share limited personal information, such as contact information, with volunteers who are assisting Team PH and who require the information as part of their duties. Volunteers who are entrusted with personal information execute confidentiality agreements with us prior to receiving any such information.

Social Media. If you engage with us on a public, social media platform, you may be sharing your Personal Information with your friends associated with your social media account, to other website users, and to your social media account provider, in connection with your social sharing activity. Please note that any information you post within these public forums or social media pages may become public and may be read, collected, and used by anyone. 

Other Uses or Disclosures of Personal Information. We may also use and disclose your Personal Information as we believe to be necessary or appropriate: (a) to comply with applicable law, to respond to requests from public and government authorities, to cooperate with law enforcement, or for other legal reasons; (b) to protect our rights, privacy, safety or property, and/or that of our users or others; and (c) for permissible public health purposes. In addition, we may use, disclose or transfer your information to a third party in the event of any reorganization, merger, sale, joint venture, assignment, transfer or other disposition of all or any portion of our organization (including in connection with any bankruptcy or similar proceedings).

Your Consent. In addition to the other disclosures described in this section, we may disclose your information as you expressly authorize.

Remember – Your Information is Very Personal

Be advised that some information provided to us is deeply personal, including stories of your Pulmonary Hypertension journey; or information for online Team or Patient profiles such as hobbies and identifying photos; or a medical diagnosis, prescriptions, treatment histories and treatment locations, physician encounters, or other medical information; or past, present, or future geographic locations (for example, people will share an upcoming event they plan to attend, including date and place); or photo(s) showing persons, their friends, family, and meaningful and identifiable physical structures and buildings; or even stories of birth and death, including dates and locations. We may include this information, if provided, in the content we make public on our web platform.

Donor Privacy

We take donor privacy seriously and we will not sell or trade donor names or Personal Information. We will not send our donors mail or marketing emails on behalf of other organizations.

Cookies and Other Tracking Technologies

We use cookies, other tracking technologies, and third-party analytics tools to better understand who is using our Platform and how users are interacting with it. These tools collect information sent by your device or the Platform when you interact with it. We use this information to analyze and track data, determine the popularity of certain content, and better understand our users’ online activity, among other things.

Google Analytics. We use Google Analytics to understand who is using the Platform and how they are using it. Google Analytics uses cookies to collect and store information about website usage, like what websites are visited, where users click, how much time users spend on each webpage, IP address, type of operating system, location-based data, device ID, search history, and other information. Pixel Tags and Similar Technologies. Pixel tags (also known as web beacons and clear GIFs) may be used to, among other things, track the online actions of Platform users (including email recipients), measure the success of our marketing campaigns and compile statistics about usage of our online resources.


Targeted Outreach. We may work with third-party service providers to target our advertisements and outreach to you based on your preferences or internet activity information, in certain cases combining data we collect from you through online cookies and other technologies with other de-identified data (such as a hashed, non-readable email or postal address). In addition, we may use third-party service providers to display content-specific advertisements to you after you have left our Sites when you go to other websites. These third-party service providers may use cookies, web beacons, or similar technologies, to tailor advertisements to interests shown by browsing our Sites or other by visiting other websites. We do not control the third-party service providers’ tracking technologies or how they may be used. The collection of your information or use of technologies by third-party service providers are subject to those third parties’ privacy policies, not ours. Retention

Team PH retains Personal Information that we receive for as long as necessary to fulfill the purposes for which the information was collected, to provide our services and resources, to pursue legitimate business purposes, to enforce our agreements, and comply with all applicable laws.

Notices and Disclosures

Children’s Privacy. Team PH does not knowingly request or collect information from any person under 13 years of age without parental consent. If you believe that your child has submitted Personal Information to us without your consent and you would like this information to be restricted or removed, please contact us using one of the methods provided in “How to Contact Us” below.

Links to Third-Party Websites. Our Platform may contain links to third-party websites, with privacy practices different from this Privacy Policy. This Privacy Policy does not address, and we are not responsible for, the privacy, information or other practices of a third-party website or service. In addition, we are not responsible for the information collection, use, disclosure or security policies or practices of other organizations, such as Facebook, Apple, Google, Microsoft, or any other social media platform provider, operating system provider, wireless service provider or device manufacturer, including with respect to any Personal Information you disclose to other organizations.

Information on Third Parties. If you disclose any Personal Information relating to other people to us, you represent that you have the authority to do so and to permit us to use the information in accordance with this Privacy Policy.
